Health care provider involvement is vital to the success of the Colorado Heart Healthy Solutions program.
Thus, CHHS works hard to build strong relationships with our local health care providers in order to achieve our goal of improving cardiovascular health in Colorado. In an effort to maintain regular communication with the health care providers in our CHHS communities, the CHHS medical staff conducts onsite lectures and distributes monthly emails that contain evidence-based Clinical Challenges, Quick Facts or community-specific report of CHHS data. Below are the emails that have been distributed to our providers as well as resources for providers and educational materials for patients.
